STATEMENT OF DECLARATION

By submitting this form, I the above noted casual employee of 2G Events Ltd (t/a Pratis Barns), have asked to be paid for casual hours worked via BACS and not through normal payroll services. I am aware that the business is set up for payroll services

with HMRC for PAYE and National Insurance but have declined to have my casual wages paid in such a manner. This is at my request, and I am not under any due influence from the business to be paid in such a way.

As such, I take full responsibility for reporting to HMRC my wages through self-assessment or tax return, if applicable.

I understand that not having my wages paid through payroll means I am required to make my own National Insurance contributions to HMRC. I understand that if I do not pay these then I may lose out on contributory benefits, such as statutory sick pay or the state pension, that depend on a national insurance record.

I also understand that not declaring my gross pay to HMRC may mean I am liable for tax and national insurance and confirm that I will make arrangements for these myself.
